Direct answer

The short answer

A shift meal sign-up needs the service date, site, shift or break group, attendee identifier, meal participation and dietary requests. Count and confirm each service separately. An employee working more than one meal period may need several portions, while dietary meals remain within each shift's total.

Use the free kit

Collect replies. Prepare the meal count.

Copy questions into your organization’s Google or Microsoft Form, or use the printable sheet. Then enter or import the replies in LunchLink’s private browser planner. No sales email is required.

Set up Google Forms or Microsoft Forms

Quick setup: create a form in your organization’s account, copy the questions above, choose appropriate response access and add your organizer contact and deadline. For one respondent’s several services, request one response per date/service. The planner does not host a shared response form.

Optional Google generator: download the Apps Script, open a new project at script.google.com, paste the file and run createLunchLinkMealForm. It creates a new draft form and response spreadsheet in your account. It does not publish the form, email attendees or send data to LunchLink. Google asks you to authorize Forms and Sheets access; review the script first. Review questions, sharing, response settings and menu choices before publishing. Organization policies may restrict scripts.

Microsoft Forms: copy the question bank into a new form. Review who can respond and whether names are recorded. Use its response export for Excel; rename columns to match the CSV template before importing into the browser planner.

Keep responses private: do not publish response summaries or a named allergy list. Check whether respondent sign-in is suitable for staff without work accounts. Compare the responses with your invitation list to find non-responders. Use your own email or Teams process for reminders; this toolkit does not automatically send messages.

At a glance

Key takeaways

  • Use a separate date/site/shift label for every service.
  • Provide paper or supervisor-assisted entry for staff without company email.
  • Confirm delivery windows and handling with the kitchen for each drop.
01

Start with actual break groups

A warehouse's HR shift names may not match its meal breaks. Ask the operations lead which groups will eat together and when. Label services in a way the receiving team understands, such as Brampton / afternoon break, and keep that label consistent in the form and final order.

Black Friday and Boxing Day can involve changing rosters and staggered breaks. Use the same template with the correct event preset; confirm working dates, actual attendance and each eating window rather than assuming everyone on the payroll needs the same meal.

02

Collect a portion for each intended meal

Ask Will you eat this meal? for every date and service. One employee may need breakfast and lunch, while another only joins the second meal. Distinguish meal portions from unique employees when reporting the program total.

For teams without work email, share a QR code pointing to the organizer's responder form and offer assisted entry. Use organizer-assigned IDs consistently. Do not publish a named list of allergies on a break-room wall.

03

Choose a format for the service window

Compare individually allocated meals with shared service based on break length, setup space and how food will be received and held. Individual packaging may simplify allocation, but packaging alone does not establish allergen safety or solve hot/cold holding.

Ask the supplier to confirm a handling plan for each delivery. Do not schedule a morning drop for a later shift without an agreed storage and service arrangement. Separate drops may have different minimums, delivery fees and receiving requirements.

04

Turn the sign-up into a service brief

Review confirmed and tentative counts by service, then agree meal choices and dietary arrangements. Send a brief with the service label, meal date, eating time, requested arrival window, portion total and receiving instructions.

LunchLink's existing Black Friday, Boxing Day and holiday planners can help develop the catering request. A completed sign-up sheet does not reserve a restaurant, guarantee a delivery time or establish the final price.

Research and limitations

How this guide was prepared

Prepared from the primary references below and LunchLink's workplace meal-planning workflow. Examples and worksheet data are fictional. Template fields and counting rules are original planning recommendations, not a clinical assessment or a claim of legal compliance. Software interfaces and organization settings can change; check the actual form before sharing.

Important limits

  • Collect a separate response for each person and meal service. Reconcile duplicate IDs before using totals.
  • An allergy or dietary request is not an accepted accommodation. Confirm the complete meal and preparation arrangements with the chosen kitchen.
  • Forms and tracker copies remain under the organizer's control. Keep named responses private and share only the information required for the meal.
  • LunchLink catering availability, minimums, prices and delivery windows in Toronto and the GTA are confirmed in a written quote.
